In person home visits are currently available for Bartow, Cobb, and Paulding county Georgia (travel charge for areas outside of these radius')
After booking appointment you will be sent two forms to complete. Both the Patient Consent and Intake form must be completed PRIOR to visit.
Before your appointment:
Please start a full feed for baby 2-3 hours before the start of our appointment in hopes of your little one being hungry for the appointment. We would like for your baby to be hungry but not "hangry" but if the feeding schedule does not allow this to happen it will be fine. We can still perform several assessments.
Please have available:

A log of the past 24 hours (stools and feeding frequency)

All devices you are currently using for feeding (nipple shield, etc.)

Your breast pump and all attachments you are currently using.

If you are supplementing because of latching difficulties, please have a bottle with expressed breast milk or formula available.

Office Visits: Located at 201 W Memorial Drive Dallas, Ga. Nursing pillows and throw pillows are provided during visit. The lactation exam room has limited space however, there is a waiting area for any persons that may need to accompany you to your appointment.
Home visits: Due to allergies, the nature of pets with a new baby and other issues I ask that ALL pets be put away upon consultants' arrival. We ask that you have a clean, flat surface to place our infant scale.
Virtual visits: Leche and Smiles will supply the secure link for our consultation once consent and intake forms have been received. We ask that you are in a well lite area with reliable internet connection.

After the appointment
A personalized care plan will be discussed and explained. You will receive your written care plan via email within 24 hours after your appointment. You can email or text regarding questions or concerns and I will respond as soon as possible (usually within 4 hours)
Patients' charts and care plan will be sent to your baby's pediatrician. Charts can also be sent to your OB/GYN as well as any specialty providers upon your request.
Telehealth, office, and home visits all come with a complimentary 1 week follow-up communication via email or phone. This allows you to ask any additional questions in order to determine if follow up care is necessary.
